1. I agree and represent that I submit this Waiver and Release of Liability voluntarily and of my own free will.
2. I have been informed that I should consult a health care professional before starting this Pilates program, and I am aware that Pilates is an exercise method and should not be substituted for medical care in the case of injuries or illness.
3. I fully understand that: (a) Pilates involves risks and dangers including, but not limited to, serious physical injury, paralysis and death; (b) these risks and dangers may be caused by my own actions or inactions, the actions or inactions of others participating in Pilates at the Studio or the negligence of the Released Parties named below; and (c) there may be other risks and social and economic losses either not known to me or not readily foreseeable at this time. I VOLUNTARILY ASSUME ALL SUCH RISKS WITH FULL KNOWLEDGE AND UNDERSTANDING OF THE DANGER AND RISK INVOLVED WITH MY PARTICIPATION IN PILATES AND MY USE OF THE STUDIO.
4. I am unaware of any physical or mental condition that would: (a) prevent me from safely participating in Pilates, or (b) endanger my health or safety or the health and safety of others due to my participation in Pilates. I understand and agree that none of the Released Parties may be held liable or responsible in any way to me or my heirs, successors or assigns, for any injury, death or other damages that may occur as a result of my participation in Pilates or as a result of the negligence of any participant or party, including the Released Parties.
